Company, transitioning to insurance company known as Care Improvement Plus
I have experience with a disease management company that specializes in
Diabetes, CHF, COPD, ESRD. I applied disease management interventions to
identified patient population on a daily basis I conducted initial assessments of
patients for participation in the Medicare Health Support Program. I served as a
key contact/expert for enrolled patients, their caregiver and physicians for:
